Community Health Worker Navigator (Screenwise)
Compensation: $21/Hour
Provide individuals with culturally appropriate information for the purpose of reducing barriers to low-cost or no-cost breast and/or cervical cancer screening services.
Job Duties
- Provide screening and health system services as well as breast and/or cervical cancer screenings to program participants.
- Enroll a minimum of 80 individuals in the program for each of the Clackamas, Multnomah, and Washington counties.
- On an annual basis, provide at least three culturally appropriate outreach and education screening services for breast and cervical cancer.
- Conduct intake, assessment, and orientation for ScreenWise services.
- Assist individuals diagnosed with cancer to enroll in Project Access Now (Tri-County) or Project Access Network (Marion and Polk Counties).
- Provide care coordination and navigation to individuals who have either individual or employer health insurance.
- Schedule screenings with network ScreenWise providers.
- Regular attendance is required.
- Other duties as required.
Job Qualifications
- Bilingual, bi-cultural English and Spanish required.
- A high school diploma or equivalent is required.
- Community Health Worker certification or experience is preferred.
