Family Support and Connections (FSC)

The Family Support and Connections (FSC) program at Northwest Family Services offers short-term, personalized support to help families strengthen their foundation and overcome life’s challenges.

We serve Clackamas County families through both ODHS referrals and self-referrals. You can learn more about this program and even fill out a self-referal on the ODHS website.

Our team works with each family to:

• Identify strengths, clarify needs, and understand barriers
• Develop a customized plan to meet immediate needs and long-term goals
• Link families to helpful resourcesincluding parenting education, youth services, food, clothing, transportation, medical support, and more
• Provide culturally responsive services in English and Spanish
• Support families over a 2–3 month period to promote stability and growth

Each family is partnered with a dedicated Family Support and Connections Specialist who listens with care, helps navigate complex systems, and offers meaningful guidance every step of the way.

Eligibility:

• Services are free and confidential
• Available to residents of Clackamas County only
• Open to self-referrals and families referred by Oregon DHS
• The family is not enrolled in active Child Welfare services
