Job Summary

Northwest Family Services (NWFS) offers a variety of prevention services within East Multnomah County. In partnership with Centennial Schools this project will serve middle school and high school students, ages 13-18, who are at risk for truancy. The Education Support Coordinator, along with MSW interns, will provide screening, referral, and individual case management focused on academic, mental health/social support, and family interventions. Evidence-based, trauma-informed, and mindfulness groups will improve students’ decision-making, distress-tolerance skills and help reduce repeated discipline referrals. Summer programming will be offered to at-risk students to assist with transitions to middle school and high school. Youth of color will be prioritized as racial disparities exist with respect to attendance, academic achievement, and graduation rates. This project is preventing early disconnection from school by providing support during and throughout middle school and high school. It is also aimed at improving quality of life and health by addressing other risky behaviors associated with truancy.

Duties and Responsibilities

  • Provide 1:1 case coordination and case management to at least 20 CHS and CMS students at-risk for truancy. Weekly Check in/Check out approach or more intensive services depending on student’s needs. One of the most important roles of the Education Support Coordinator is building a trusting relationship with the student and their family to strengthen their connection to school.
  • Conduct GAIN-SS screenings with all students receiving case management or participating in groups and refer appropriately.
  • Conduct at least one meeting with family of case management youth
  • Provide evidence-based groups to 30-40 students, including two high school boys’ empowerment groups (once a week for 12 weeks); a middle school life skills group (once a week for 12 weeks); and a middle school mindfulness group for students in-school suspension (twice a week, 30 mins per session).
  • Track attendance and academic progress of case management youth
  • Refer to and facilitate the Family Empowerment Class
  • Refer families to NWFS for insurance enrollment
  • Maintain up-to-date documentation on case files.
  • Become knowledgeable of current school and district policies and protocol regarding truancy/ crisis management planning.
  • Use of SAMSHA’s Screening, Brief Intervention, and Referral to Treatment (SBIRT) model with students at-risk of substance use and mental illness. Decrease drug and alcohol use by instituting the Teen Intervene curriculum with students and family members.
  • Attend assigned trainings and participate in collective impact meetings and other meetings with school staff
  • Be able to assess for suicide risk, follow protocol, refer, and follow-up
  • Provide program updates during supervision and as requested by Program Manager. Provide case files and records to Program Manager for review at the end of each quarter or as needed.
  • Facilitate a strength-based atmosphere when working with students that is conducive to academic success and positive social skills.
  • Participate in afterschool events including parent-teacher conferences, multicultural nights, “Movin' on Up night”, and “Unity Week”, etc.

Additional Responsibilities

  • Provide support to other prevention programs in East Multnomah County, including the Four Cities Peer Court, Family Empowerment, and Stay On Track.
  • Demonstrate and maintain positive relationships with NWFS and school staff as well as community members.
  • Provide positive role modeling for the students.
  • Effectively communicate about NWFS, its mission, and programs to community partners and participants.
  • Attend staff meetings and development as required.
  • Accept the guidance and supervision of the project manager.
  • Conduct other duties as assigned.

Job Qualifications

  • Minimum of a Bachelor’s Degree in social work, psychology, behavioral health or related fields. Equivalent experience of 3 or more years will be considered.
  • Work experience in school settings (preferred)
  • Fluent in English and Spanish (preferred)
  • Able to work occasional evenings and weekends (e.g. once a month)
  • Training and experience related to counseling/treatment principles for individual, family and group counseling; adolescent development; family systems theory, and addictions treatment/intervention
  • Familiar with positive youth development and trauma-informed approaches
  • Ability to work effectively with youth
  • Reliable, dependable, and accountable; follows through on commitments
  • Excellent oral and written communication skills
  • Strong organizational skills and follow-through
  • Ability to work independently and with others
  • Proficiency in web-based research and Microsoft Office software
  • Commitment to maintain appropriate confidentiality as outlined by NWFS guidelines
  • Ability to pass criminal background check

Working Conditions

Work is performed typically in an office environment and involves periodic use of a computer and phones. Must be able to lift up to 30 lbs.

Salary: $16.50/hr to $18/hr includes pro-rated sick, holiday, and vacation time
