Adolescent Substance Use/Abuse Services

Empowering Youth for a Brighter Future

The goal of the Transitions program is to provide convenient, confidential, and respectful school-based prevention services to adolescents and their families in Clackamas and East Multnomah Counties. All services are culturally competent and trauma-informed and utilize evidence-based practices and curriculum. Our approach is built on the values of family, motivation, respect, resilience, and problem-solving. Our programs are designed to offer safety and support by providing individuals with structure, accountability, encouragement, honesty, and caring feedback. We provide support to youth and families in learning new information and skills while practicing new ways of thinking and communicating.

While we do provide treatment services in a school setting across Clackamas County and East Multnomah County, we also have the ability to provide services within the community at safe, non-traditional community locations as well as our local offices in Clackamas and Gresham.

School Suspension?

Have you or your child been suspended from school due to a violation of the School District Policy around substance use on campus and been suspended? The Transitions program meets assessment and treatment requirements for all Alcohol or Drug Suspensions in various schools within Clackamas and East Multnomah County. Minor in Possession?

Transitions does work closely with the Juvenile Department in both Clackamas and East Multnomah County and meet the treatment requirements for most diversion programs and many probation programs. A direct referral from the Juvenile Department is not required however, we will request information from you, as well as a release of information to make sure we are able to provide the Juvenile Department with the necessary information to ensure compliance needs.
