Who We Are: Passionate Advocates for Youth

At East Multnomah Transitions, our dedicated team is committed to providing comprehensive and compassionate substance use treatment, prevention support, and peer support services to the youth and families of East Multnomah County. Our goal is to empower young individuals aged 11-19 through a strength-based and harm-reduction model, enabling them to make informed choices about substance use and lead fulfilling lives.

We are driven by our passion for supporting youth with the respect and compassion they deserve. We believe in helping young individuals find their voice, recognize their value, and unlock their full potential. Our team members bring expertise, empathy, and a commitment to making a positive impact on the lives of the youth we serve.

Youth Referral Form - East Multnomah County

This referral form is for students and youth who live, attend school, or receive services in East Multnomah County. Our Transitions teams work with young people to provide support, resources, and connections that help them reach their goals.

If you are a parent, guardian, school staff member, or community partner, you can use this form to refer a youth to our program. Once submitted, a team member will review the referral and reach out to begin the process of connecting the youth with services.
