School-Based Alcohol and Drug Treatment Services Focusing on Adolescent Prevention and Treatment in Clackamas County

The goal of the Transitions program is to provide convenient, confidential, and respectful school-based treatment and prevention services to adolescents and their families. Our program is culturally competent, trauma-informed and utilizes evidenced-based practices. Our approach is built on the values of family, motivation, respect, resilience and problem- solving. Our programs are designed to offer safety and support by providing individuals with structure, accountability, encouragement, honesty, and caring feedback. We provide support to individuals in learning new information and skills while practicing new ways of thinking and interacting.

School Suspension?

This program meets assessment and treatment requirements for all Alcohol or Drug Suspensions in various Clackamas County Schools (see locations).

Minor in Possession?

This program meets assessment and treatment requirements for most diversion programs and many probation programs within the Clackamas County Juvenile Department.
